Clergy Guidelines
Select one of the Application Form links below. You must print and complete the application in full. If any part of this application is not filled out completely, LCP will return the application to our office and your approval will be rejected. In order for us to process your application, please print clearly and adhere to the following:
- FULL name must be given, including middle name, not just middle initital.
- Address must be full, including state and zip code.
- Phone number must include area code.
- Applicant and Witness need to sign and date in each other's presence.
- Write N/A in any category that does not apply to you.
Please carefully read the entire Visiting Guidelines before completing the Application for Clergy Approval. Then mail the completed application with your endorsement letter to:
